How to set up an employee probation feedback form?

The employee probation feedback form is a straightforward tool that enables supervisors or managers to share their feedback on an employee's performance during the probationary period.


At Keka, we offer a convenient option for employees who have either finished their probation process or are approaching its completion. You can gather feedback from both the employee and their reporting manager by utilizing our customizable feedback form. You have the choice to either use our pre-designed template or create a form from scratch based on your specific requirements.


Open the Org section of your portal and navigate to Employees, then go to Probation. Here open the Probation Feedback Form section.



Here, you will find two sections, For Reviewers and For Employees

In the For Reviewers section you have the question to take the feedback for an employee's performance in the probation period.

To add a question to this feedback form, click on +Add Question



A new question field will appear where you can put the question statement (1) in the field provided and if required, add a description (2) also.

Then select the type of response field (3). (Likert scale is recommended for rating-type questions) 

Click on Save (4) to add the question to the form.


The Likert scale is a rating scale that measures people's attitudes or opinions. It uses response options like "Strongly Agree" to "Strongly Disagree" to capture individuals' viewpoints. It's a simple and effective way to gather quantitative data on people's perceptions or attitudes.



Similarly, you can open the For Employee section and click on +Add Question to add a question to the feedback form.



In any of the section, you can click on Show more to get the option to modify a question.



You can use the Edit and Delete icons to update or remove the question as per your requirement.
